How do I get Immunizations?

Bring your immunization record if you have it, as well as your insurance card. We accept most insurance and medicaid. Immunization certificates are also available at our office. Please contact either our Pineville (606) 337-7046 or Middlesborough (606) 248-2862 locations.

How do I obtain my food safety card?

The Bell County Health Department currently offers its Food Handler’s training class as an online internet based course. Please click here for instructions.

How do I obtain a copy of my birth certificate?

For a copy of a birth certificate, please contact the Frankfort Health Department at (502) 564-3970. A birth certificate form request can be found by clicking here.

Who is eligible for the WIC Program?

The WIC (Women, Infant, and Children) program is available to women who are pregnant, breastfeeding, postpartum, or with an infant/child up to 5 years old, who have a household income level at or below 185 percent of the poverty level.
